Teenagers go through a lot of changes while facing important choices about life
and testing their independence.
HealthPass for Teens is a program especially for teenagers, aged 14 to 17, with a
comprehensive web-based teen health assessment. Your teen can learn more about good nutrition,
being more physically active, and can meet privately and confidentially
with a health consultant to discuss their concerns.
Your teen visit includes:
- Health screening measurements
- Teen health risk assessment questionnaire
- Meeting with a health consultant
HMSA's HealthPass for Teens is available to eligible HMSA members at no cost.
