What is a health risk assessment?
It is a convenient, comprehensive questionnaire that evaluates your health and lifestyle-related behaviors
and identifies your health risks. When completed, you will receive a private and personalized health
profile and lifestyle score, a wellness action plan as well as access to many online tools, resources
and interactive lifestyle-management programs.
What are the available questionnaire formats? Is it available in other languages?
You may access HealthPass' Health Risk Assessment questionnaire online in this website. Or, if you prefer to
complete the HRA on paper, simply contact our HealthPass staff. The paper questionnaire is also available
in Ilocano or Japanese.
Do I need to make a HealthPass appointment first?
Not necessarily. You may complete an online or hardcopy questionnaire at any time. If you wish to schedule
a HealthPass appointment, please contact us at 432-9220 on Oahu or toll-free 1 (800) 525-6548.
What is a lifestyle management program?
HealthMedia®'s interactive lifestyle programs are helpful tools that can help you meet your health-related
goals. Programs include; weight management, nutrition, smoking cessation and stress management.
What is HealthPass?
HealthPass is HMSA's comprehensive health promotion and prevention program. The program features a health risk
assessment, screenings, and interactions with health professionals. Over 20,000 HMSA members participate in
HealthPass each year.
Do I have to pay for my HealthPass appointment?
No. It is available as a benefit at no cost to eligible HMSA members.
What does a HealthPass screening appointment involve?
HealthPass will...
- Evaluate your lifestyle, health habits and health risks
- Measure your blood pressure, cholesterol, glucose and body composition (height, weight, body mass index, waist and body fat)
- Help you develop a personal plan to achieve wellness goals
- Refer you to healthy lifestyle programs, health education classes and other services.
Do I need to fast for my HealthPass appointment?
Yes, for the adult glucose screening there is a 4-hour fast. Do not eat or drink anything other than water 4 hours
prior to your appointment. If you have diabetes and are taking medications, you do not need to fast.
What else do I need to do to prepare for my HealthPass appointment?
- Please bring your photo I.D. and HMSA coverage card.
- Avoid heavy exercising 4 hours prior to your appointment.
- Continue taking medication that is prescribed by your doctor.
- Refrain from drinking any alcohol for 24 hours prior to your appointment.
- For the body fat analysis, drink 2-4 cups of water within 2 hours of your appointment and wear comfortable clothing.

What is HealthPass for Teens?
HealthPass for Teens is a program especially for teenagers, aged 14 to 17, with a comprehensive web-based
teen health assessment. The visit includes a health screening and measurements, a teen health risk assessment
questionnaire and a private, confidential meeting with a health consultant.
Who's eligible?
HealthPass for Teens is available to eligible HMSA members, aged 14 to 17, at no cost.

What is HealthPass at the Worksite?
HealthPass' health risk assessment program is available at the worksite, statewide and is subject to
participation minimums. The onsite HealthPass benefit includes:
- A personal assessment of each employee's health habits and lifestyle.
- A biometric screening which includes:
- Height, weight, and Body Mass Index (BMI)
- Waist measurement
- Body fat analysis
- Blood Pressure
- Blood cholesterol (total and HDL)
- Blood glucose (fasting)
- Resting heart rate
- A personal health coaching session conducted by a health professional, incorporating the results of the lifestyle
assessment and biometric screening.
Who is eligible?
All of this is available as a plan benefit at no additional cost for HMSA members. HMSA spouses are also welcome, if space
is available. We will also provide the health screening service to non-HMSA members, if requested, for a nominal fee, per person.
What is the participation minimum required to bring this service out to my worksite?
A minimum of 25 participants is required to bring HealthPass to the worksite. However, we would be happy to work with you if the
minimum cannot be met. Companies with 50 or more participants will receive an in-depth aggregate report, which assesses the
company's most prevalent health conditions, risk factors and biometric indicators.

What is health coaching?
It is a telephonic or one-to-one health coaching service offered to HealthPass participants who are interested in additional
follow-up and support for healthy lifestyle changes.
Who are the health coaches?
Our health and wellness coaches are professionals with varying backgrounds in the health sciences field. They are trained
to assist you with establishing individualized goals and making healthy lifestyle changes.

What is nutrition counseling?
The nutrition counseling program is designed to support HMSA members by empowering them to make better dietary choices for
healthier living. The focus of the program is to direct members to achieve healthier outcomes based on their specific and
individual nourishment needs.

What are your Health Education classes?
Currently, there are seven classes offered through our Health Education Program on a variety of topics including physical
activity/exercise, nutrition, stress management and healthy aging.
Is there a cost to attend these classes?
As an HMSA member, you may attend these classes at no charge. There is a fee for non-HMSA members.
